Jasmine had a skin flare up when a pup, mentioned in a different thread made worse by vet applying Advocate, which led to four bare patches with sores on her back.
We saw a great skin specialist in Melksham, Wiltshire. She's never had a really bad reaction last 4 years, maybe it was 1 off maybe not.
Upshot skin vet put Jasmine onto:
Hills Z/D Ultra dry food, still has this with lamb mince. Probably doesn't need it but kept her on it as didn't want to risk it. [We buy it from the internet as it's about expensive as it gets.]. She has 'Supadog' sensitive lamb and rice kibble as treats and James Wellbeloved 'CrackerJacks'.
Malaseb shampoo [POM-V], antibac and antifung. Shampoo worked straight away, within days. Fuciderm gel [carbomer gel for dogs] to apply to her spots/sore areas, always keep some just in case.
First vet gave me Seleen shampoo [didn't work].
Obviously speak to your vet but this has worked for Jasmine, who has stayed a healthy Akita.
Not advising you to do so but have used 'piriton' on Jasmine [child's dose] after she ate 2 pork chops off a plate and scratched and scratched, it worked.
Jasmine gets the odd treat from others and picks up what she can find like all dogs, not kept on a religious anti allergy diet.

it is piriton that Thor has been prescribed, he is on 16mg 3 x a day, we are not paying for it - yet, still waiting for insurance to pay up so it is adding to the mounting bill at the vets! we have paid the first few hundred out but the bill is topping £1300 now so they are being great and claiming direct since his skin biopsy's.
he is also on salmon & rice diet, as he has alergies to meat but we dont seem to be seeing any difference, its just so frustrating!
the vets dont seem to think his paws & skin are conected, but im not so sure.

Is it your usual vet or a skin specialist? Vet's I guess to a lesser degree are like GP's.
My vet referred me quite early into Jasmine's treatment, in our case it paid dividends.
Google the Z/D Ultra, [chicken based, with chicken protein removed]. Might be similar to the salmon and rice?
We added lamb mince as it was the only meat/fish that we'd never given her, but that was as she wouldn't eat the kibble [nearly three weeks on and off].

its our regular vet, i think as alergy tests are coming back positive they do not want to refer him, its not something he has said to us, but until the insurance decide to pay up we cant really ask to be referred because if the vet doesnt think it necasary the ins definately wont pay!

they hadnt! i think mainly because it isnt on his body just around his eyes & lips, and possibly why he is chewing at his joints - though im not convinced this is down to itching - but im definately trying the one you sugested, can you buy it over the counter like at pets at home?

Hun, the Vet that Keo is seeing is a skin specialist, He is the vet that will be logging on to see if there are any questions for him. Keo has to go back about his paws next week but i'll see if we may be able to draw Thor's plight to his attention chick. It's been going on for so long. xxxx
